Director of Community Outreach and Health Service

As a member of the Central Community Health Centre (CCHC) Management Team, the Director of Community Outreach and Health Services will report to the Chief Executive Officer (CEO) and work in close collaboration with the Primary Health Care Team, the Administrative Team, and all other staff plus volunteers. The Director will:


  • Provide leadership to coordinate and supervise the development, implementation, monitoring and evaluation of community services and interdisciplinary programs;

  • Provide leadership to the budget planning, monitoring and reporting processes;

  • Be responsible for formulating and managing a comprehensive communications and social media strategy;

  • Be responsible for formulating, implementing and managing a comprehensive fund development program; and  

  • Take their turn working in the CCHC’s Mobile Clinic.


Your duties and responsibilities in this role will include, but are not limited to:


Human Resource Management


  • Hire, supervise and evaluate interdisciplinary team members, including but not limited to a Mental Health Worker, Systems Navigator, Community Youth Workers, Health Promoter, Social Workers, Dietician, Chiropodist, and other allied health professionals and volunteers;

  • Develop and implement mechanisms to ensure regulated and non-regulated staff work to their full capacity;

  • Co-ordinate student placements within the program team.


Program Management


  • Provide leadership in the identification of program needs and priorities in consultation with internal and external service providers, community groups, community members and others as appropriate;

  • Provide leadership in the development, implementation and evaluation of strategies and programs to meet identified needs;

  • Develop and implement mechanisms to ensure effective service delivery, including the implementation of a quality assurance program and the development of policies and procedures for all program functions;

  • Determines staffing, space and other resource requirements as required for effective program delivery

  • Develop and manage annual budget submissions;

  • Develop, co-ordinate and supervise methods to collect and analyze health services data and performance indicators; report quarterly to the management team on target achievement;

  • Develop funding proposals as appropriate; manage the outputs of funded projects and write reports as required.


Communications and Community Relations


  • Provide leadership in the development, implementation and evaluation of a strategic communications strategy for the CCHC;

  • Identify key relationships that should be nurtured to benefit and promote CCHC, and network widely to establish and maintain community partnerships for the effective delivery of services;

  • Represent CCHC on multi-disciplinary committees and community agency working groups as assigned by the CEO;

  • Participate in public events to promote CCHC programs and services, as assigned by the CEO;

  • Advocate on behalf of CCHC and CCHC clients, especially relating to the Social Determinants of Health and their relationship to health and wellbeing.


Organizational Management


  • Contribute to the effective delivery of services and the efficient administration of the organization, including participating in the development of CCHC policies and procedures;

  • Provide advice and assistance to the CEO by keeping him or her informed of all aspects of program team operations;

  • Act on behalf of the CEO in his or her absence when requested to do so;

  • Provide support to Board and Staff committees as assigned by the CEO;

  • Perform other related duties as assigned.

  • Master’s level degree in health administration or a related discipline or an undergraduate degree and five to seven years progressive management experience;

  • A minimum of five years management experience preferable in a community health or primary health care setting;

  • Three to five years of community program delivery experience, with progressive responsibility, preferably in a community health setting;

  • Experience in the areas of primary health care, community development, health promotion and disease prevention;

  • Human resource management skills, including experience hiring staff, managing an interdisciplinary team, team building, team planning, facilitation, performance evaluation, conflict resolution and problem solving;

  • Thorough knowledge and proficiency in program development, delivery and evaluation;

  • Administrative skills, including project management, scheduling, health records management, developing and managing budgets, completing reports, operational planning, preparing grant applications and funding proposals, developing policies and procedures;

  • Experience in the implementation and monitoring of quality assurance systems and programs;

  • Experience in research, statistical analysis and/or accreditation would be an asset;

  • Must have a valid Ontario Driver’s Licence and access to a vehicle.

  • Hours of work: 37.5 hours per week; flexibility to work some evenings and/or weekends is required
